Seeing a new doctor can be an anxious experience sometimes. Especially if you’ve never had chiropractic care before. Paperwork to fill out, new faces and names to learn, and getting to know your way around the office; this can sometimes be overwhelming.

At Back Bay Wellness our wish is to remove any cause for unnessesary tension and make you feel at home. On this page you will be able to take care of items necessary for your visit prior to arriving, in the comfort of your own home.

In order to make your first trip a pleasant one (and shorter!), please take a moment to save the following forms to your computer and fill them out at home in your free time. When you arrive for your first appointment, just give them to the receptionist and relax in one of our massage chairs while we put your data into the computer.
